Oak Plains Academy has a unique opportunity for a Masters' Level Case Manager to join our team of professionals who support our youth living in Oak Plains Academy foster homes across Middle Tennessee. The ideal candidate will enjoy the benefits of a flexible schedule, diverse responsibilities, and smaller caseloads which allows more focused time to build stronger connections with the youth served.

Located on 20 acres of rolling hills in a rural setting, about 45 minutes from Nashville, Tennessee, Oak Plains Academy offers psychiatric residential treatment services to children and adolescents ages 5–17, struggling with emotional and behavioral issues. The Oak Plains Academy campus features a full-size gymnasium, separate academic building, and four dormitories. Our wooded campus provides an excellent opportunity for outdoor recreational activities such as fishing, hiking, softball and basketball. Additionally, Oak Plains Academy provides therapeutic treatment and intensive case management support to youth, from infant to 17 years old, residing in our Foster Homes. The Foster Home Case Manager will be responsible for providing in home trauma based therapy and intensive case management and coordination of services to youth and adolescents.

Key Responsibilities:

  • Coordinates treatment-planning sessions, notifies all necessary parties, manages the meeting, and documents the progress and goals discussed as per documentation policy.

  • Develops a Family Service Plan, monitors implementation of the plan, and locates all services and placements a child and/or family may need while enrolled in the continuum.

  • Advocates for assigned children, acts as a resource for assigned foster parents, and provides training and consultation to foster parents when appropriate.

  • Ensures implementation of treatment plan; consults with Program Director regarding issues with resources necessary for plan implementation.

  • Documents progress and problems, maintains contacts with the custody department and Resource Managers, revises the Family Service Plan as needed, maintains ongoing contacts with the child and/or family, plans and implements them.

  • Participates in on-call schedule to provide on-call services to foster parents. Reports and documents any on-call activities the next business day. Calls Continuum Director after hours for consultation as needed.

  • Reports any non-compliance with regulatory agencies or policies and/or any suspicions of abuse or neglect to Continuum Director or to proper authorities.

  • Visits each assigned child per policy or as needed. During visits to assigned home, completes the "Contact Log" sheet as per documentation policy.

Qualifications

REQUIREMENTS:

  • Minimium of a Masters in Psychology, Counseling, Social Work, or related Human Services Field.

  • Minimum of one year Case Management experience working with youth.

  • Valid TN State Driver License.
