Seeking a full-time organized, detailed oriented, pragmatic, and collaborative community health worker to join the community health team based in our Mecosta County office.

General Responsibilities

• Through home visits and in clinic and community settings, works 1 on 1 with clients to secure information such as medical, psychological, and social factors contributing to client's situation.
• Records information in database, works to identify and navigate resources to support the client and eliminate barriers to good health.
• Advocates for clients and acts as a liaison between clients and other service providers.
• Helps clients and families understand, accept and follow medical recommendations.
• Provides healthcare enrollment services, Medicaid Outreach, and DHD#10 services outreach in community settings.
• Covers Mecosta County.
