The Care Management Discharge Planning Assistant will support the Medical Center's Care Management department, delivering high-quality care to patients based on assessed needs, established standards of care and according to policy and procedures. Education and Experience Requirements:

• Education: High school diploma or GED (required), bachelor's degree in social work or nursing (preferred)
• Experience: 1 to 2 years (preferred)
