PRIMARY CARE PARTNERS Physician, Geriatrics Division in GRAND JUNCTION, Colorado

Job Summary:

The responsibility of this position is to provide high quality medical care that is person centered and evidence-based.

Duties/Responsibilities:

Provides direct patient care to adults in the subacute rehab and long-term care setting, as well as the possibility of assisted living residences, personal homes and office. Assesses patient health by interviewing patients; reviewing staff notes; performing physical examinations; and ordering appropriate diagnostic tests. Create care plans with patients and their decision maker. Documents necessary medical information into the EMR in accordance with coding, billing, COPIC, and Primary Care Partners expectations. Responds in a timely manner to questions in person, via telephone or fax, from facility staff. Instructs and counsels patients around: Overall wellness and health maintenance Counseling for emotional and/or mental health problems. Geriatric syndromes End of life planning Works with and directs clinical staff on triage, care management, and refill requests. Maintains safe and clean working environment by complying with procedures, rules, and regulations. Geriatric Nurse Practitioner, Geriatric Executive Assistant

General Requirements:

Able to work independently; detail oriented with good organizational skills; able to multi-task and change priorities/focus; good communication skills, team oriented; the ability to work effectively with a diverse group of personalities; good decision making skills.

Work Experience:

A minimum of __ years' experience as a physician. Experience in treating geriatric patients in SNF and long-term care settings preferred.

Education Requirements:

Valid CO Medical license and DEA number.

Board Certified (Or CAQ) in Geriatrics or Hospice and Palliative Medicine preferred but not required

Working Environment:

This job operates in a professional medical office environment and facility setting. This role routinely uses standard office equipment such as computers, phones, photocopiers, scanners, filing cabinets and fax machines in addition to patient care services.

Physical Demands:

The physical demands descripted here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear. This is both a sedentary and walking/standing role; it would require the ability to maneuver patients, bend or stand, and sit on a stool/chair as necessary. Specific vision abilities required by this job include close vision, distance vision, and ability to adjust focus.

Position Type/Expected Hours of Work

This is a full-time position 40 hours. Occasional a.m. meetings and evening work may be required as job duties demand. Participate in on-call pool.
