We are seeking a Care Navigator Level 1 to join our multi-disciplinary team in Guthrie at our new Urgent Recovery Center (URC). In this role, some of your responsibilities will be working collaboratively with clients, families, support systems, and community partners to support health behavior change and achieve treatment goals. You will provide crisis management services, conduct assessments, develop care management plans, and coordinate care across various treatment milieus.

Qualifications

• Bachelors degree in a human services field
• ODMHSAS Case Management certification (or ability to obtain within 60 days of hire)
• Minimum of one (1) year experience in a mental health-related field or healthcare setting preferred
• Proficiency in MS Word and intermediate general computer skills
• Excellent verbal and written communication skills
• Access to reliable transportation and valid Oklahoma drivers license
• CPR, First Aid, and Therapeutic Options certifications (or ability to obtain within 30 days of employment)
