Opportunity Join and collaborate with a palliative care physician, and other palliative care team members, in an active practice to deliver palliative care and related health care services to patients and families. Perform bedside palliative care patient/family evaluation. Assist with diagnosis, treatment, and management of acute and chronic health conditions. Consult with palliative care physician or designees as needed, inform primary physician of services provided and collaborate with other physicians as needed. Assist in all facets of care coordination for palliative care referrals. Hours are M-F 8:00am - 5:00pm No call expectations initially, but a call component may be added in the future Qualifications Graduate of accredited NP program Current WI RN and APNP license Current DEA Other certifications as required Minimum of 5 years nursing experience; clinical nursing care in oncology, hospice, geriatrics, intensive care preferred.
