NorthCare Behaviorial Center ISS Home Based Therapist in Oklahoma City, Oklahoma

This job was posted by https://okjobmatch.com : For more information, please see: https://okjobmatch.com/jobs/3001673 The Intensive Safety Services (ISS) therapist provides in-home therapeutic services to families who are at risk for child welfare court involvement. Team members work with the families for an average of 4-6 weeks with sessions 3-5 times per week; therapist carry an average of three families. All service activities are designed to support the outcomes of safety, permanence, and well-being of Oklahomas children. Position is supervised by Program Manager.

Essential job functions include, but are not limited to:

Manages a caseload of three to four family systems referred to us by Oklahoma Human Services, in person at least three times per week with highly complex cases requiring intensive clinical service and extensive professional diagnostic and treatment experience.

Implements evidence-based interventions in the context of individual, family, and/or group therapy.

Provide on-call and after hours support to families to support family stabilization and to help prevent disruption.

Connects families with community resources and services and assists with overcoming barriers to accessing services.

Assists families with special funding to bridge gaps for existing and emerging needs that threaten family stability.

Performs intakes and diagnostic assessments to include clients symptoms, behavior, functioning, history, needs, goals, and preferences.

Delivers services which address behavioral healthcare, physical healthcare, and social needs that are recovery focused, strengths based, culturally competent, client centered, trauma informed, and co-occurring capable.

Collaborates with Comprehensive Home-Based Services (CHBS) and community partners to foster stabilization, ensure service transfers as indicated, and provide care navigation.

Provides crisis management services, which may include face-to-face or telephone interviewing of the client; assessment of lethality to self or others; assessment of available support systems; and assistance in arranging for appropriate intervention, including hospitalization. In the cases where a potential for harm exists, will complete appropriate assessment tools and, if needed and in consultation with management will facilitate referral to an appropriate level of care. Immediately seeks consultation with supervisors regarding crisis situations, including potential safety concerns.

Obtains and maintain required credentialing including but not limited to licensure or under supervision status and Alcohol and Other Drugs Treatment Practitioner (AODTP) status.

Obtains and maintains understanding and competency in any other instruments required by agency contracts, payors and/or commonly used by mental health/substance use professionals.

Participates in staff consultations, case staffing with consumers, contract partners, and other services providers to coordinate care needs.

Supports the monitoring and tracking of outcome measures for clients.

Maintains clinical records in a timely manner, as required by agency policy, contracts, funding sources and accreditation authorities. Complete all required forms in a manner consistent with requirements.

Maintains professional competency in the area under ones responsibility through appropriate readings, attendance at workshops and training. Comply with all requirements to maintain professional certification(s).

Knowledgeable regarding: Oklahoma Human Services ISS contract, service provisions and electronic record requirements; NorthCares electronic record system and public folders; CCBHC requirements, and other relevant contracts or public policy impacting service population.

Promotes a warm and engaging organizational culture in line with Northcare values, trauma informed care, and health and wellness.

Provides integrated services that are welcoming, ngaging, and accessible.

Returns phone calls and cell text pages promptly. Check and respond to email daily. Maintains consistent work attendance and contacts supervisor on timely basis for needed late arrivals or absences.

Completes all NorthCare required courses and training and complies with all agency policies and procedures, including but not limited to documentation deadlines and productivity expectations.

Performs any reasonable work requested that falls within the qualifications but not specifically described in work performed.

Position requires a masters degree in social work, psychology or a related human services field and licensure in the State of Oklahoma, in one of the mental health disciplines or under board approved supervision for licensure. Licensed LCSW, LPC, LMFT, LADC-MH, LBP, or under supervision required; if licensed, employee must complete continuing education requirements required for licensure maintenance.
